How do I get appointed with Secure Horizons?
Follow these steps…
1. Complete
and sign the Contract and Appointment Application for Medicare Advantage and Prescription Drug.
2. Complete
and sign the Agent Agreement for both the Medicare Advantage & Prescription Drug.
3. Copy of your
individual state insurance license.
4. Completed
W-9 form.
5. Copy of Errors
& Omissions declaration page.

What is the next process?
Once we receive the
above information you will be notified and instructed on the process for completing your Certification test.
When can I write business?
Once all contracting
has been received by our office and the certification has been completed on line, business can be written and submitted.
